Case Management for Long-term Hospitalization by Medical Aid Beneficiaries : Suggestions for Successful Operation and Stabilization

Abstract
Objective: The purpose of this was to explore the current status and issues regarding case management for long-term hospitalization by Medical aid beneficiaries and to provide suggestions for its improvement.

Methods: Literature on management for long-term hospitalization by Medical aid beneficiaries were retrieved from databases and thoroughly analyzed. Key words of ¡®medicaid¡¯, ¡®case management¡¯, and ¡®long-term hospitalization¡¯ and their combinations were used as search terms. A total of 61 articles were included in the analysis.

Results: Case management for long-term hospitalization by Medical aid beneficiaries is a complex phenomenon taking place under the social contexts where the government's social welfare and health care policy, local governments, private hospitals, beneficiaries and their families are entangled. For successful operation of the case management for long-term hospitalization, reinforcing supervision of health care providers, and expansion and improvement of local community resources to provide proper after-discharge care. Also improvement in overall underlying conditions for case management should be preceded.

Conclusion: Supportive environments for case management should be established for its successful operation and stabilization. To do so, it is essential to make orchestrated efforts from multiple stakeholders including the government, local communities, health care providers in acute and long-term care settings, and beneficiaries and their families.
